How your information will be used
When you arrive at Parkside you will be asked for information about your medical history. This helps us to provide appropriate care for you and meet your needs.

The information provided to us will be shared with your GP and other healthcare professionals involved with your care (such as district nurses). Your information is only passed on when essential to your continuing care. At your request, our staff can update a named friend or relative on the progress of your treatment, please let us know who you would like to be informed when you arrive.

Our staff may use your information for other purposes including:

  • Review the care provided for teaching or training, or investigating complaints
  • Managing and planning the services we provide the NHS, eg: ensuring we can meet patients’ needs, recording performance

Everyone working for the Trust – and anyone receiving information from us – has a legal duty to keep information about patients completely confidential. Where possible, details that identify you will be removed. The Medical Director monitors patient confidentiality in his role as Caldicott Guardian.
